To apply for enrollment in the 2025–2026 school year, please complete all of the following:

Google Enrollment Application — First step for all students.

Procare Application Form — Sent by email if a spot is confirmed for your child.

State of Florida DCF Child Care Application for Enrollment — Due within 10 business days of submitting your Google Enrollment Application.

DH Form 680 Florida Certification of Immunization — Available from your pediatrician; due within 30 days of your child’s first day.

DH Form 3040 School Entry Health Form — Available from your pediatrician; due within 30 days of your child’s first day.
A non-refundable $250 application/activity/supply fee will be charged through our Procare school management system once your application is received and a spot is confirmed. This fee secures your child’s place in the program.
- You may also pay by check in the preschool office.
- If the application fee is not submitted by the due date on your Procare invoice, your child’s spot may be forfeited.
- Church members pay a reduced $200 application/activity/supply fee.
If a program is full, you may choose to be placed on our waiting list. Being on the list does not guarantee a spot, but earlier placement increases your chances. The waiting list is updated annually.
